- Rag rating all referrals sent to Bury Adult Care Services
- Contacting all Service Users who meet the criteria for CAD Triage to ensure safety and wellbeing
- Liaising with professional services – CMHT, OPCMHT, Police, Hospital, GP’s some cases require a multi-disciplinary approach
- Locating relevant services – Food Banks, BAME Groups, Men/Women/LGBT+ Support Groups.
- Carrying out initial Needs Assessments – this is often for the purpose for STH to locate appropriate housing.
- Undertaking home visits where required
- Recognising and taking appropriate action when risk is identified
- Supporting Enquiry Officers – calls received that are concerns will then be passed to CAD Triage
- EDT Notifications – carrying out further enquiries or sign posting to the relevant team
- Often SF1 forms do not meet SG Team criteria these are then sent back to CAD Triage to be managed
