Reporting to: Lead Clinical Pharmacist / Clinical Directors

 

Merco Medical Staffing is seeking a dedicated Clinical Pharmacist to work for our Client across the Primary Care Network in Cambridge. This role is an exciting opportunity to be part of a dynamic team and contribute to providing high-quality patient care.

  Key Responsibilities:
  • Medication Review & Optimisation: Address pharmacy queries, review medication, and advise on appropriate alternatives during availability issues.
  • Care Home Medication Reviews: Conduct medication reviews for care home residents.
  • Long-Term Condition Clinics: Manage medication for patients with chronic conditions.
  • Medicines Reconciliation: Process discharge letters and reconcile medications post-hospitalisation.
  • Unplanned Admission Prevention: Help prevent unplanned hospital admissions through medication management.
  • Medicines Information & Education: Provide medicines education to patients and healthcare teams.
  • Prescribing Systems & Policies: Support the development and implementation of prescribing policies.
  • Medicines Safety: Promote safe medication practices across the network.
  • Collaboration: Liaise with community and hospital pharmacies to ensure seamless patient care.
  • Reports & Audits: Run regular audits and reports to monitor medication safety and effectiveness.
  • Additional Duties: Take on any other responsibilities as required by the practices.
