Leaving Care Personal Advisor
Service care Solution are currently recruiting for a Leaving Care Personal Advisor in Bedfordshire.
The Leaving Care Personal Advisor will provide essential support, advice, and guidance to care-experienced young people as they transition out of the care system.
Pay rate - 14.99 PAYE / 18.46 Umbrella
Main Responsibilities
As a Leaving Care Personal Advisor you will be responsible for:
- Manage a caseload of young people leaving care, ensuring that Pathway Plans are developed, monitored, and reviewed regularly in partnership with the service user.
- Complete Pathway Assessments in a timely and collaborative manner, ensuring active participation from the young person, carers, social workers, and other professionals.
- Provide consistent and ongoing support to care leavers, assisting them in accessing resources and services, and offering advice and guidance on a range of issues that may affect their transition into independent living.
- Work in partnership with other professionals, including social workers and carers, to ensure a comprehensive and coordinated approach to the young person's support.
Requirements:
- Experience in a similar role
- Full UK Driving Licence and access to own vehicle
- Enhanced DBS
