Job Description:

My client is seeking a dedicated and experienced Care Plan Coordinator to join their team. The successful candidate will be responsible for developing comprehensive care plans for young people entering their care setting. This role requires a strong understanding of behavioural support, return home plans, and the creation of positive activity programs.

Key Responsibilities:
  • Develop and write detailed care plans for young people entering the care setting.
  • Create behavioural support plans tailored to individual needs.
  • Develop return home plans and interview guidance for children in care who have run away.
  • Establish and maintain a mentoring system, ensuring effective referral processes and planning with appropriate services.
  • Design and implement guidance for providing positive activities for young people, including:Sports, Outdoor education, Creative and digital arts, Craft-based activities, Music and theatre, Sensory and other appropriate activities
  • Collaborate with social workers to understand expectations and requirements prior to service commencement.
  • Maintain accurate and up-to-date records of care plans and activities.
  • Ensure compliance with all relevant regulations and standards.

Requirements:

Proven experience in creating care plans, behavioural support plans, and return home plans.
Experience in developing interview guidance for children in care who have run away.
Strong understanding of mentoring systems and referral processes.
Ability to design and implement a variety of positive activities for young people.
Excellent communication and interpersonal skills.
Strong organizational and time management abilities.
Understanding of social workers expectations and requirements.
Relevant qualifications in social work, psychology, or a related field are desirable.
