Mental Health Outreach Worker

Brook Street Social Care are working with a reputable mental health charity in Cardiff to join a close-knit well-established team. If you are passionate about making a real difference in people's lives - then apply today! The role will involve supporting adults with mental health needs in the community and their own homes.

Key Responsibilities:

  • Provide one-to-one support for adults with mental health conditions within their own homes and local communities.
  • Manage a caseload of vulnerable adults, conducting assessments and reviews of their mental health and wellbeing.
  • Assess information from a variety of sources, ensuring the appropriate services and support are identified and delivered.
  • Facilitate service user involvement, ensuring their voice is central to all aspects of their care and support.
  • Address housing management and organisational issues that may impact service users' mental health or wellbeing.
  • Work collaboratively within a supportive and dedicated team to deliver high-quality care and support services.

Requirements:

  • A valid driving license and access to your own vehicle, with business insurance.
  • Experience in supporting adults with mental health conditions is preferred, but we will consider applicants with a strong commitment to making a difference.
  • Strong communication and organisational skills, with the ability to assess and review complex information.
  • Ability to work independently and as part of a team, with a proactive approach to problem-solving.
