Duties and Responsibilities
- To arrange and monitor appropriate services in line with individual care plans.
- To provide advice and practical help.
- To liaise with voluntary and statutory agencies.
- To complete annual care plan reviews and or reassessments.
- To participate in team meetings and duty rotas.
- To undertake these duties in line with Council Policies and Procedures.
- To undertake training and self-development and to contribute to this process in others.
Corporate Requirements

As a term of your employment you may be required to undertake such other duties and/or times of work as may reasonably be required of you, commensurate with your grade or general level of responsibility within the organisation.
Although you will be provided with a base, you will be required to work from various locations in accordance with the needs of the role.
Knowledge and Experience
- Experience of working with older people and adults with a physical disability through direct contact in a working environment.
- Ability to work independently and as part of a team.
- Ability to make relationships with se5rvice users and carers.
- Administrative ability
- Good communication skills
- Organisational skills
- Understanding of the needs of elderly and or disabled people.
- Willingness to undertake appropriate training courses.
- Empowerment of disabled and or elderly people and their carers.
- Understanding of the needs of elderly and or disabled people.

Desirable
- Full Valid Driving Licence
- Educated to GCSE standard or equivalent
- NVQ/GNVQ Level 3 or 4 in Social Care
- Experience of working with staff from a wide variety of agencies.
- Experience of working within a hospital setting.
