About the Role: Our client, Cardiff Council, is seeking an experienced and dedicated Social Worker to join their Adults Learning Disabilities Team. This role focuses on supporting adults with learning disabilities to live independently and achieve the best possible outcomes, working within a person-centered approach.

Key Responsibilities:

  • Manage a caseload of adults with learning disabilities, ensuring their needs are assessed and met effectively.

  • Develop, implement, and review care and support plans tailored to individual needs.

  • Work collaboratively with service users, families, carers, and partner agencies to deliver coordinated services.

  • Advocate for individuals, ensuring their rights and choices are respected.

  • Conduct safeguarding assessments and interventions in line with legislation and policies.

  • Provide professional advice, guidance, and support to individuals and their support networks.

  • Maintain accurate records and contribute to service improvements and policy development.

Essential Requirements:

  • Qualified social worker (Degree in Social Work or equivalent) with current Social Care Wales registration.

  • Experience working with adults with learning disabilities in a social care setting.

  • Strong understanding of The Social Services and Well-being (Wales) Act 2014, safeguarding procedures, and relevant legislation.

  • Excellent communication and assessment skills to develop effective care plans.

  • Ability to work independently and as part of a multidisciplinary team.

  • Commitment to promoting independence, inclusion, and person-centered care.
