We provide personalised outcome-based support to enable individuals with significant care and support needs to live safely, happily and successfully in the community.

We operate seven apartment-based care homes located in Castleford, Garforth, Snaith, Bradford, Roundhay, Pudsey and Rastrick. Each of our homes provides supportive accommodation for 6-9 individuals within their own homely flat-based settings.

Job Purpose -

The PBS Practitioner is responsible for developing, implementing, and overseeing Positive Behavioural Support (PBS) strategies to improve the quality of life for individuals with learning disabilities, autism, and complex needs. The role aims to reduce challenging behaviours, promote independence, and ensure that support plans are person-centred and effective.

Key Responsibilities -

  • Develop & Implement PBS Plans:
  • Create Positive Behavioural Support (PBS) plans based on thorough assessments of individual needs.
  • Conduct Functional Behavioural Assessments (FBA) to inform PBS plans.
  • Regularly monitor and update PBS plans to reflect changes in behaviour or support needs.

Collaboration & Person-Centred Planning:

  • Involve individuals, families, and key stakeholders in developing support plans.
  • Tailor plans to meet the unique needs, preferences, and goals of each individual.
  • Ensure support plans align with best PBS practices and promote autonomy.

Training & Guidance:

  • Provide staff with training and guidance on PBS principles and practices.
  • Support the delivery of PBS-related training sessions, including de-escalation techniques, risk assessment, and person-centred care planning.
  • Deliver PBS and physical intervention training sessions.
  • Deliver PERMA Training.
  • Complete direct observations when required.

Data Collection & Analysis:

  • Collect, analyse, and report data on behavioural incidents and the effectiveness of PBS interventions.
  • Use data to refine support plans and inform decisions.

Collaboration with Teams:

  • Work closely with Principal PBS Practitioners, Operations Managers, Registered Managers, and the broader team.
  • Participate in team meetings, practice workshops, and governance reviews to share insights and discuss progress.
  • Participate in PERMA workshops and support services to review actions.
  • Participate in Service PBS and PI reviews and support services to review actions.

Risk Management & Compliance:

  • Implement and monitor risk management strategies balancing safety and autonomy.
  • Regularly review restrictive practices, working towards their reduction or elimination.
  • Ensure compliance with regulatory standards and company policies.

Continuous Improvement & Quality Assurance:

  • Participate in safeguarding, incident review, and quality assurance activities.
  • Contribute to continuous service improvement by identifying development areas and implementing best practices.

Review & Analyse Incident Reports:

  • Review incident reports to highlight common themes and trends.
  • Ensure records are maintained accurately in the Nourish electronic care planning system.

Desirable Qualifications and Experience

  • Degree in Psychology, Behavioural Sciences, or a related field.
  • Professional certification in Positive Behavioural Support (PBS) or equivalent.
  • Extensive experience with individuals with learning disabilities, autism, and complex needs.
  • Proven experience in developing and implementing PBS plans.
  • Strong knowledge of functional behavioural assessment and intervention techniques.
