Job Overview:We are looking for a dedicated Band 6 Occupational Therapist to join the acute assessment team at Leighton Hospital. This role involves providing top-tier care and assessment for patients with various acute conditions. You will work within a multidisciplinary team to support rehabilitation, promote quality of life, and empower patients to reach their potential.

Key Responsibilities:

  • Conduct comprehensive assessments of patients in a general acute setting.
  • Develop and implement individualized treatment plans.
  • Collaborate with physiotherapists, nurses, consultants, and other healthcare professionals to ensure holistic patient care.
  • Monitor and evaluate treatment progress, adjusting plans to meet evolving patient needs.
  • Educate and support patients and families to facilitate a smooth transition from hospital to home.

Key Requirements:

  • HCPC registration as an Occupational Therapist.
