Role Summary:Chorley CMHT is looking for a dedicated Band 6 Mental Health Practitioner to provide individualized, holistic mental health support for service users from initial assessment through to discharge. This role focuses on needs assessment, collaborative care planning, and recovery-focused intervention, while also acting as a key liaison across multidisciplinary teams to ensure a comprehensive approach to care.
Key Responsibilities:
- Conduct comprehensive needs assessments, develop care plans, and support users through their recovery journey.
- Implement and evaluate individualized care plans in collaboration with service users, carers, and external partners.
- Perform clinical risk assessments and manage risks using person-centered strategies.
- Provide therapeutic interventions and advocate for individuals, facilitating their safe integration into community services.
- Support and provide supervision for nonqualified staff and promote best practices across the team.
- Collaborate with external agencies, including social services and community-based organizations, to enhance care continuity.
- Lead on new patient assessments and ensure smooth case management transitions.
Essential Skills and Experience:
- Strong clinical assessment and care planning abilities.
- Proficiency in therapeutic interventions and risk management strategies.
- Basic understanding of the Mental Health Act.
- Ability to work effectively across multidisciplinary teams and organizational boundaries.
- Prior experience working in mental health and community settings preferred.
