Job Overview:We are looking for an experienced and motivated Band 6 Physiotherapist to join the community team at University Hospitals Coventry and Warwickshire NHS Trust. In this role, you will deliver high-quality physiotherapy services to patients in the community, managing a caseload of individuals with a variety of musculoskeletal and rehabilitation needs. You will be responsible for conducting assessments, developing treatment plans, and supporting patients with their rehabilitation to improve mobility and independence.

Key Responsibilities:

  • Manage a community caseload, providing physiotherapy assessments and treatment plans for patients with musculoskeletal and rehabilitation needs.
  • Work collaboratively with multidisciplinary teams to ensure the best outcomes for patients.
  • Provide advice and education to patients and their families about self-management and rehabilitation strategies.
  • Develop and implement evidence-based interventions for a diverse range of conditions.
  • Maintain accurate and up-to-date patient records and documentation.
  • Contribute to the development of services and quality improvement initiatives within the team.

Key Requirements:

  • HCPC registration.
  • Minimum 6 months of post-graduate experience in community physiotherapy or relevant clinical settings.
  • Strong experience in musculoskeletal rehabilitation and complex case management.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Driving license and access to a car for community visits.
