About the Role:

We are seeking a skilled and experienced Band 6 Community Psychiatric Nurse (CPN) to join our Community Mental Health Team (CMHT) across the East Midlands area. This role is crucial in supporting the function of the Locality Pathway Allocation Coordinator by providing clinical oversight for the triage of routine referrals. You will work closely with Band 4 clinical support assistants to ensure appropriate pathways are identified for patients.

Key Responsibilities:

  • Provide clinical oversight for the triage process, ensuring that routine referrals are accurately assessed and directed to appropriate pathways.
  • Collaborate with the Locality Pathway Allocation Coordinator to streamline the referral process and improve patient outcomes.
  • Supervise and support Band 4 clinical support assistants in their daily tasks related to referral triage.
  • Conduct comprehensive assessments of patients' mental health needs and develop individualized care plans.
  • Liaise with multidisciplinary teams to coordinate and deliver high-quality care.
  • Maintain accurate and up-to-date patient records, ensuring compliance with relevant policies and procedures.
  • Participate in regular team meetings and contribute to service development initiatives.

Requirements:

  • Registered Mental Health Nurse (RMN) with a valid NMC pin.
  • Proven experience as a Band 6 CPN or similar role within a mental health setting.
  • Strong clinical assessment and triage skills.
  • Excellent communication, organizational, and interpersonal skills.
  • Ability to work effectively as part of a multidisciplinary team.
  • Familiarity with the Community Mental Health Team (CMHT) processes and pathways.
