Service Care Solutions is currently recruiting for a Community Psychiatric Nurse (CPN) to join a multi-disciplinary Age Independent Memory Assessment Service within the NHS in Warwickshire.

Role Overview:The post holder will play a crucial role in providing assessment and diagnosis for individuals experiencing cognitive decline. This service aims to determine the appropriate interventions while adhering to the principles of the "Living Well with Dementia" strategy in the least restrictive environment.

Key Responsibilities:The successful candidate will be responsible for delivering a range of interventions in accordance with operational policy, including but not limited to:

  • Memory Assessment: Conduct comprehensive assessments to evaluate cognitive function and memory.
  • Timely Diagnosis: Ensure prompt diagnosis of cognitive impairments to facilitate early intervention.
  • Pharmacological Treatment: Administer and monitor pharmacological therapies as indicated by assessments.
  • Post-Diagnosis Support/Signposting: Provide ongoing support and guidance to patients and their families following diagnosis, connecting them to relevant services and resources.
  • Monitoring/Review: Regularly review patient progress and adapt care plans as necessary to meet changing needs.

Qualifications and Skills:

  • Valid registration as a nurse with the NMC
  • Previous experience in mental health nursing, preferably within memory assessment or dementia care
  • Strong communication and interpersonal skills
  • Ability to work effectively in a multi-disciplinary team
