Band 6 community / HEF dietitian for 5 months for home visits and NH interventions

We are seeking a skilled and compassionate Community Dietitian specializing in Home Enteral Feeding (HEF) to join our team. This role involves working within the community to provide expert dietary assessment, planning, and management for patients requiring home enteral feeding support. The HEF Dietitian will work collaboratively with healthcare professionals, families, and caregivers to support patients’ nutritional needs and optimize their health outcomes.

Key Responsibilities
  • Patient Assessment and Care Planning:

    • Conduct comprehensive nutritional assessments for patients requiring enteral nutrition, including those with complex clinical needs, chronic illnesses, and disabilities.
    • Develop individualized nutrition care plans and enteral feeding regimens, selecting appropriate formulas and feeding methods.
    • Adjust feeding plans based on regular assessments, lab results, and clinical indicators to meet ongoing patient needs.
  • Patient and Caregiver Education:

    • Educate patients, caregivers, and family members on managing enteral feeding at home, including the safe handling of equipment, administration techniques, and hygiene practices.
    • Provide tailored nutrition and lifestyle advice to optimize patient health, encourage dietary variety, and prevent complications associated with enteral feeding.
