EHCP and Transitions Officer
  • Location: Croydon
  • Job Type: Full-time, Term time only
  • Hours: 8.00am – 4.00pm, 37.5 hours per week
  • Accountable to: Headteacher

We are supporting a school in seeking an EHCP and Transitions Officer to join their dedicated team. The school is a Special School for students with Profound and Multiple Learning Disabilities, severe sensory impairment, and complex medical needs. 

Day-to-day of the role:
  • Coordinate and administer the EHCP and review processes, ensuring compliance with statutory timelines.
  • Organise and invite stakeholders to review meetings, facilitating and chairing as needed.
  • Take minutes for Annual Review meetings and other required meetings.
  • Liaise with Local Authorities, Social Care, and Health Professionals, as well as the school’s multidisciplinary team.
  • Coordinate transitions into the school for new students, including tours, assessments, and induction processes.
  • Update school prospectus and materials for prospective parents.
  • Collaborate with teachers and families to co-produce transition plans for students.
  • Provide transition support for families exiting the setting.
  • Develop external links with services such as work experience placements and other settings.
  • Host and lead training for families on transition-related topics.
  • Track destination data and provide family support and signposting.
  • Seek feedback on transitions from stakeholders.
  • Act as Deputy Designated Safeguarding Lead.
  • Assist with general school office duties and administrative support for the headteacher.
  • Work collaboratively with finance and HR to ensure smooth administrative functions.
Required Skills & Qualifications:
  • Knowledge of SEND and EHCP procedures.
  • Excellent communication skills, both written and verbal.
  • Understanding of safeguarding children and vulnerable adults.
  • Familiarity with Data Protection and confidentiality principles.
  • Understanding of SEND code of practice and disability legislation.
  • Comprehension of the school’s ethos, values, and the transitions process.
  • Ability to establish and maintain positive relationships with staff and external support.
