Personal Advisor - Leaving Care Team

About the Role:

As a Personal Advisor, you will play a crucial role in helping care leavers navigate the challenges of adulthood. Your responsibilities will include:

  • Building strong relationships with care leavers and understanding their individual needs and aspirations.
  • Implement and review Pathway Plans, through direct work with the young people and their families
  • Providing comprehensive support on a range of issues, including housing, education, employment, health, and finance.
  • Developing personalized support plans and monitoring progress towards goals.
  • Advocating for care leavers' rights and ensuring they have access to appropriate services.
  • Collaborating with other professionals, such as social workers, housing officers, and health professionals, to coordinate support.
