Description

-Assessing and reviewing renablement needs and goals, for people who are discharged from hospital via the Discharge to Assess (D2A) Pathway.-Assessment of social care needs-Assessing renaablement needs and goals-Reviewing needs and goals-Use Social care IT

-Knowledge of basic safeguarding legislation and processes.

-NVQ Level 2 or equivalent.
