We Care Bespoke is recruiting for a Social Worker to join the Adult Learning Disabilities and Physical Disabilities Team in Darlington
In this role, you will work closely with adults who have learning and physical disabilities, helping them to achieve greater independence, well-being, and access to tailored support services.

Key Responsibilities:

  • Conduct assessments to determine the needs of adults with learning and physical disabilities, and develop person-centered care plans that promote independence and quality of life.
  • Work collaboratively with health professionals, housing services, and other agencies to provide a coordinated approach to care.
  • Advocate for individuals to ensure their rights are respected, and that they have access to the services and resources they need.
  • Ensure compliance with relevant legislation, including the Care Act and Mental Capacity Act, and adhere to safeguarding procedures.
  • Support adults in transitioning to or maintaining independent living arrangements, including making referrals to appropriate community resources.

Requirements:

  • Qualified Social Worker with valid Social Work England registration.
  • Experience working with adults with learning disabilities and/or physical disabilities.
  • Strong understanding of relevant legislation, including the Care Act, Mental Capacity Act, and safeguarding practices.lls.
  • Ability to work collaboratively within a multidisciplinary team.
  • Commitment to promoting equality, diversity, and inclusion in all aspects of work.
