About client:

We are currently seeking a Senior Physiotherapist to join our team at RDH. This role is perfect for an experienced physiotherapist with a passion for delivering high-quality care and contributing to the discharge planning process.

Key Responsibilities:

  • Provide expert physiotherapy assessment and treatment for patients on acute inpatient wards.
  • Lead and coordinate the discharge planning process, ensuring safe and effective patient transitions from hospital to home or other care settings.
  • Work collaboratively with multidisciplinary teams to ensure holistic care delivery.
  • Develop and implement individualized rehabilitation plans based on patients' needs and clinical conditions.
  • Supervise and mentor junior physiotherapists and support staff.
  • Maintain accurate and up-to-date patient records, ensuring compliance with professional standards and best practices.
  • Participate in regular clinical supervision and continuous professional development activities.

Key Requirements:

  • HCPC registration is essential.
  • Recent experience in an acute inpatient ward setting is required.
  • Discharge planning experience is essential, demonstrating the ability to plan, assess, and execute safe patient discharges.
  • Ability to work autonomously and as part of a multidisciplinary team.
  • Excellent communication, organizational, and clinical skills.
  • A commitment to delivering high-quality patient care and achieving positive outcomes.

Desirable:

  • Postgraduate qualifications in physiotherapy or a related area.
  • Experience in leading or contributing to departmental audits or quality improvement projects.
