Key Responsibilities:

  • Physiotherapy Assessments: Conduct comprehensive assessments with acute stroke patients, formulating and delivering individualised treatment plans.
  • Treatment Delivery: Provide skilled physiotherapy interventions, including patient handling and mobility training.
  • Discharge Planning: Identify appropriate discharge options, co-ordinate safe discharges, and recommend/adapt mobility aids and equipment.
  • Collaboration: Work closely with a multi-disciplinary team (MDT) to provide individual and joint treatment sessions for stroke patients.
  • Mobility Assessments: Complete complex mobility assessments and prescribe a range of mobility aids to improve patient function.
  • Documentation: Maintain accurate and concise medical records and provide clear onward referrals.

Essential Qualifications & Experience:

  • Hold a Physiotherapy degree (MSc or BSc) and full HCPC registration.
  • A minimum of 12 months NHS experience in an acute stroke or neuro setting.
  • Evidence of relevant stroke/neuro-specific training.
  • Computer literate and able to use electronic medical records systems.

Key Skills & Knowledge:

  • Clinical Skills: Proficiency in conducting physiotherapy assessments, treatment planning, patient handling, and mobility aid prescription.
  • Stroke Knowledge: Strong understanding of acute stroke, National Stroke guidelines, and physiotherapy models of practice.
  • Communication Skills: Effective, compassionate communication with patients and collaboration with MDT members; ability to document patient progress clearly.
  • Time Management: Ability to manage a dynamic and high-volume caseload, prioritising patients based on clinical need.
  • Ethical & Professional Practice: Uphold patient autonomy, respect diversity, and maintain confidentiality at all times.
  • Complex Assessments: Ability to assess and interpret patterns of movement and complete complex mobility assessments.
