About the Role

We are looking for a passionate and experienced Social Worker to join Derby City Council’s Preparing for Adulthood (PfA) Team. This team works with young people aged 14-25 with additional needs, supporting their transition from childhood to adulthood. The role focuses on helping young people achieve greater independence, ensuring they receive the right support to prepare for adult life, including education, employment, health, housing, and community participation.

Key Responsibilities
  • Work collaboratively with young people, families, and professionals to develop and implement person-centered plans that support young people in achieving their aspirations and independence.
  • Undertake assessments of young people under the Care Act 2014, ensuring needs are identified, and appropriate services are in place.
  • Support transition planning for young people with Special Educational Needs and Disabilities (SEND), working alongside education, health, and care professionals to ensure seamless service delivery.
  • Manage a caseload of young people, providing tailored advice, support, and advocacy to meet their specific needs.
  • Work with housing services, employment providers, colleges, and health services to ensure young people have access to the resources they need to transition successfully.
  • Safeguard and promote the welfare of vulnerable young people, carrying out risk assessments and responding to safeguarding concerns in line with statutory requirements.
  • Build strong relationships with partner agencies, fostering a multi-agency approach to meeting the needs of young people.
  • Maintain accurate and up-to-date records in line with Derby City Council’s policies and procedures.
  • Attend and contribute to reviews, case conferences, and other relevant meetings to support the young person’s journey to adulthood.
