About the Role:

We are looking for a committed and compassionate Social Worker to join our Adult Social Care team. This is an exciting opportunity to make a real difference in the lives of individuals and communities. As a Social Worker, you will provide a high-quality service, supporting individuals to achieve their most independent outcomes and safeguarding their health and wellbeing.

You will work collaboratively with individuals, their carers, families, and external partners to assess needs and develop tailored care plans. You will lead the implementation of support plans, manage a designated caseload, and empower those you work with to make positive, lasting changes.

In addition to your direct work with individuals and communities, you will also provide leadership and expertise, supporting newly qualified social workers and learners.

Key Responsibilities:

  • Work with individuals, their carers, and advocates to assess needs and deliver appropriate services/resources.
  • Develop and implement support plans that address individual goals and outcomes.
  • Provide social work support for homeless individuals and families.
  • Act as a Duty Officer.
  • Undertake specialist duties, such as BIA (or equivalent), AMHP, and Practice Educator roles, if qualified and approved by DCC.
  • Support and supervise social work apprentices and trainees, providing consultation to other staff members.
  • Produce reports, assessments, and maintain accurate case records.
  • Regularly review caseloads with your line manager and ensure that the quality of service meets agreed standards.
  • Coordinate complex care arrangements, including safeguarding, through multi-disciplinary meetings.
  • Perform financial assessments, ensuring income maximisation for individuals where required.

Essential Requirements:

  • Social Work qualification and registration with the relevant regulatory body.
  • Experience in adult social care and working with vulnerable individuals and communities.
  • Knowledge of relevant legislation and policies.
  • Excellent communication, organisational, and interpersonal skills.
  • Ability to work effectively in partnership with others, including carers, external agencies, and service users.
  • Commitment to personal and professional development.

  • Line Managed by: Service Manager
  • Supervises and Supports: Newly Qualified Social Workers and other learners/employees as required.
