We are seeking experienced and compassionate Social Workers to join our team in supporting hospital discharges at Norfolk and Norwich University Hospital. The role involves working with individuals with complex needs, empowering them to make choices that promote their health, well-being, and independence.
Key Responsibilities:- Undertake person-centred assessments to help individuals make informed decisions about their care and well-being.
- Work collaboratively with individuals to create support plans that maximise their abilities and help them achieve their identified goals.
- Provide an integrated approach to assessment and care, working closely with key partners and stakeholders.
- Manage a caseload, ensuring that assessments are conducted under the Care Act and other relevant legislation, using a strength-based approach.
- Coordinate and monitor care packages and support plans, conducting reviews to assess whether the desired outcomes are being achieved.
- Lead Safeguarding Adults investigations (for Level 2 Social Workers) and promote awareness with service users and colleagues.
- Engage in inter-disciplinary relationships, encouraging integrated ways of working across services.
- Qualified Social Worker with experience in hospital discharge settings or related fields.
- Strong analytical skills and the ability to assess and manage complex cases, particularly in relation to safeguarding, capacity, and best interest issues.
- Experience in conducting Care Act assessments and managing care plans.
- Ability to build positive relationships with service users, carers, and multi-disciplinary teams.
- Excellent written and verbal communication skills, with the ability to produce reports for court proceedings, case conferences, and other forums.
