Each team across the region provides proactive and intensive support to avoid the symptoms of a mental health emergency getting worse. The aim of the service is to offer treatment and care in an individual's own home as an alternative to hospital admission.Main responsibilitiesTriaging referrals to the service and making recommendationsComplete holistic psychosocial assessments within the communityCompleting gatekeeping assessments to facilitate admission when home treatment is not felt to be a viable / safe optionAttend Mental Health Act assessments on Bluebell Place of Safety, and formulate home treatment plans if home treatment is indicatedImplementation of Risk Management and Care Plans for the effective home treatment of Service Users utilising least restrictive practice at every opportunitySupport with medication management and concordance within communityProviding telephone support to service users who ring in during times of crisisJob RequirementsMust have a mental health nursing qualification and registered with NMC and experience of working in a community mental health team. Alternatively could hold OT or social work qualification with relevant mental health experience.Must hold a full UK driving licence. DBS checked.Have significant experience working within a community setting.
