Job Description:Are you a compassionate and experienced Occupational Therapist looking to make a real difference? Join our Reablement team and help individuals regain independence and confidence after illness, injury, or hospital admission.

In this role, you’ll be part of a dynamic team, working alongside Occupational Therapy Assistants and Support Workers to empower individuals to live independently in their own homes. You will conduct strengths-based assessments, create tailored reablement plans, and work closely with customers and their families to achieve meaningful goals.

Key Responsibilities:

  • Conduct strength-based assessments with individuals in their own homes or other settings like hospitals.
  • Collaborate with customers and their families to design and implement creative reablement plans.
  • Support individuals in regaining independence and improving their quality of life.
  • Work proactively to ensure customers can achieve their goals and reduce long-term care needs.
  • Collaborate with the wider Reablement team to ensure high-quality service delivery.

Requirements:

  • A qualified Occupational Therapist with at least two years of post-qualification experience.
  • Full UK driving license (required for home visits and travel).
  • Ability to work independently and manage a varied caseload.
