- Care Planning and Assessment: Develop, implement, and regularly review individualized care plans for residents, ensuring that all care needs are met in line with their preferences and health requirements.
- Team Leadership: Supervise and support care staff, providing guidance, training, and ensuring that care is delivered in accordance with best practices and regulatory standards.
- Communication: Serve as a key point of contact between residents, their families, healthcare professionals, and other stakeholders. Ensure clear and effective communication regarding residents' care and well-being.
- Compliance and Documentation: Maintain accurate and up-to-date records of residents' care, including care plans, medical records, and daily logs. Ensure compliance with all relevant health and safety regulations and care standards.
- Coordination of Services: Liaise with external healthcare providers, such as doctors, nurses, and therapists, to coordinate the provision of additional services or specialist care when required.
- Quality Assurance: Monitor the quality of care being provided, conducting regular audits and assessments to identify areas for improvement. Implement necessary changes to enhance the standard of care.
- Crisis Management: Respond to emergencies and incidents, ensuring the safety and well-being of residents and staff. This includes managing unexpected health issues and coordinating emergency care when necessary.
- Relevant healthcare qualification, such as NVQ Level 3 in Health and Social Care or equivalent.
- Previous experience in a care home or similar setting.
- Experience in care planning, risk assessments, and coordinating care services.
