Are you a passionate and experienced Social Worker dedicated to supporting adults with learning disabilities? We're seeking a compassionate and skilled individual to join our dedicated team in Doncaster.
About the Role:
- Provide high-quality social work support to adults with learning disabilities, promoting their independence, well-being, and inclusion within the community.
- Conduct thorough assessments to identify individual needs and develop person-centered care plans.
- Work collaboratively with individuals, their families, and other professionals to achieve positive outcomes.
- Advocate for individuals' rights and access to appropriate services and support.
- Contribute to a supportive and collaborative team environment focused on client-centered care.
Requirements:
- Registered with Social Care England.
- Minimum 3 years' post-qualification experience as a Social Worker.
- Experience working with adults with learning disabilities is highly desirable.
- Excellent communication, interpersonal, and case management skills.
- Commitment to person-centered care and ethical practice.
