Job Overview:The hospital is seeking a motivated Band 6 Occupational Therapist to join its acute team. This role focuses on delivering expert assessments, personalized interventions, and effective discharge planning in a dynamic acute care environment, aiming for exceptional patient outcomes.
Key Responsibilities:- Conduct comprehensive assessments for patients in acute medical settings.
- Deliver therapeutic interventions to support recovery and independence.
- Collaborate with multidisciplinary teams to create and implement care plans.
- Ensure safe and timely discharges with continuity of care.
- Maintain detailed and accurate documentation in line with NHS standards.
- HCPC registration as an Occupational Therapist.
- At least 6 months of NHS experience in an acute setting.
- Strong clinical reasoning and decision-making skills.
- Excellent communication and team collaboration skills.
- Dedication to providing compassionate, patient-centered care.
