About the RoleWe are seeking compassionate and dedicated individuals to assist street outreach workers in supporting rough sleepers. This is an opportunity to make a real difference in people's lives by helping them access essential services and secure housing.
Key Responsibilities
- Conduct outreach activities Monday to Friday to engage with rough sleepers.
- Collaborate with agencies to identify suitable housing and support solutions.
- Advocate for clients' welfare, including healthcare and other needs.
- Manage a caseload, providing ongoing guidance until clients are securely housed.
- Maintain accurate and up-to-date case notes using CHAIN and other databases.
