Job Title: Reablement Team Adult Social Worker Location: Tower Hamlets Full-time: 35 hours per week Pay Rate: £32 per hour Are you ready to embark on an adventurous and rewarding role as a Social Worker within the vibrant Reablement Service in Tower Hamlets? This full-time position offers £32 per hour over a period of 3 months, giving you a fantastic opportunity to make a significant impact on the community. As part of the Reablement Team, you will work closely with our Admission Avoidance Discharge Service (AADS) health partners, integrating your skills into a multidisciplinary environment. What you will do:

  • Conduct comprehensive Care Act Assessments, focusing on leveraging the strengths of individuals, their families, carers, and local communities to promote independence.
  • Collaborate closely with in-house reablement officers and therapists, guiding local residents through up to 6 weeks of hands-on support in activities of daily living.
  • Address safeguarding concerns as they arise during or after the reablement process, ensuring all actions align with relevant legislation and local authority responsibilities.
  • Develop and manage support plans, advocating for a strength-based and outcome-focused approach to empower residents towards greater independence.
  • Navigate a diverse caseload efficiently, maintaining a commitment to transforming the lives of individuals through effective social work practices.
