Qualified Social Worker - Hospital environment
On behalf of our client, we are seeking a qualified and experienced Social Worker to join an Adult Social Care team, working. This role will involve working within a hospital setting, supporting individuals through discharge planning and ensuring their care and support needs are met in a safe, timely, and effective manner.
To be considered for this role, you must hold a Social Work qualification and be registered with Social Work England (SWE). Your SWE registration number will be required as part of the application process.
As part of your responsibilities, you will manage a complex and varied caseload with appropriate guidance and supervision, delivering a strengths-based approach to all assessments and interventions. You will work closely with hospital teams and community services to facilitate smooth and efficient discharge planning, ensuring individuals transition safely from hospital to their home or an alternative care setting.
Your role will also involve undertaking mental capacity and best interests' assessments, recording findings in line with legislative requirements. You will lead safeguarding enquiries under the Care Act 2014, ensuring they are completed within required timescales and that outcomes are shared with all relevant parties. Additionally, you will support individuals, carers, and families with direct work that promotes independence and well-being while addressing any identified risks.
You will participate in a duty system and ensure that all reports are accurate, up-to-date, and submitted within required timeframes. Strong communication skills are essential, particularly in complex or high-risk situations, and you will escalate issues to senior management when necessary.
This role requires close collaboration with hospital staff and other professionals to design and deliver support plans tailored to individual needs. You will ensure services are delivered effectively, within available resources, and in compliance with legislative frameworks such as the Mental Capacity Act and the Care Act.
The successful candidate will work three days a week from the office, with the remainder of the time spent in the hospital and community. A valid driver's license is essential to facilitate hospital visits and other community-based work.
