Role: Lead PractitionerService: Learning DisabilitiesLocation: DundeeSalary: £24,000 - £29,000Working towards company values, the successful candidate will deliver support to the organisation's service users meeting their specific requirements, going over and above to provide the highest standard of care. Responsibilities:*Take responsibility for undertaking initial assessments and follow ups of people we support. Create, follow and review support plans.*Manage paperwork for service user reviews and record taking in line with the data protection act.*Provide leadership to staff with on-the-job training, take lead on shift, continuously seek service development and improvement, support and supervise other staff members administering medication. *Undertake sleepovers, overnight and weekend working. *Maintain professionalism and confidentiality. *Prepare and implement risk management plans. Experience/Requirements:*The successful candidate must hold a minimum SVQ3.*Direct experience working with those who have learning disabilities and/or forensic histories. *Experience working to SSSC standards.*An understanding of the causes and effects of social exclusion. *Experience in organisational planning and record keeping. *Knowledge of safeguarding and protection. Travel within the service area will be required. The successful candidate will be required to undertake a PVG Scheme Record application. Enhanced disclosure may be required.
