Your new company

This public sector organisation based in Durham is currently recruiting for a Support Coordinator to join their team on a full-time temporary basis.

Your new role

To provide responsive, high quality, support to clients, in order to promote independent living, as appropriate.The successful candidate will be accountable for monitoring and analysing each individual's support requirements while also offering practical assistance, advocacy, and guidance as part of an efficient team to help promote independent living. Key accountabilities will include:

  • To support clients in a flexible and person centred way, utilising interventions and delivering practical tasks which address their needs and aspirations, through implementing and authorising support/ treatment plans.
  • To undertake, authorise and review individual client needs assessments.
  • To undertake, authorise and review individual client risk assessments through effective communication and rapport building with the client.
  • To be accountable for adherence to internal targets and performance measures.

What you'll need to succeed

  • A good understanding of Safeguarding
  • Experience of empowering clients to build and develop strong support networks and positive relationships
  • A proven ability to work with clients to successfully exit the service and optimising their ability
  • A proven ability to work with individuals with diverse needs
  • Experience of working across a range of sectors
