Are you a Prescribing Pharmacist with Primary Care experience?If so, my client would be delighted to have a conversation with you!Based within a great commute of Durham and Darlington, this position suit someone who already had experience within a Primary Care setting.The role can be offered full time however, if you would prefer to be part time, the role can be split to accommodate 2 Pharmacists accordingly (3 days and 2 days respectively).Salary p/a or pro rata depending on training and experience.Key Duties
  • Perform medication reviews and optimisation by conducting comprehensive medication reviews for patients to ensure safe, effective, and appropriate use of medications.
  • Prescribe and re-prescribe medications in accordance with clinical guidelines, patient needs, and organisational policies.
  • Manage long-term conditions by supporting patients with chronic diseases (e.g. diabetes, hypertension etc) by reviewing and optimising treatment plans.
  • Liaising with the MDT, communicate with hospital pharmacists or specialists to discuss complex cases or medications that require specialised knowledge.
  • The successful applicant must be comfortable to mentor trainee support staff and pharmacists.
Package and Benefits
  • Salary IRO £43,000 - £54,000 p/a OR pro-rata
  • 6 weeks holiday + BH (FTE OR pro-rata)
Key Skills and Qualifications
  • MUST be a qualified Pharmacist in the UK with GPhC registration
  • MUST hold their Independent Prescriber qualification
  • MUST have Primary Care experience
