- Perform medication reviews and optimisation by conducting comprehensive medication reviews for patients to ensure safe, effective, and appropriate use of medications.
- Prescribe and re-prescribe medications in accordance with clinical guidelines, patient needs, and organisational policies.
- Manage long-term conditions by supporting patients with chronic diseases (e.g. diabetes, hypertension etc) by reviewing and optimising treatment plans.
- Liaising with the MDT, communicate with hospital pharmacists or specialists to discuss complex cases or medications that require specialised knowledge.
- The successful applicant must be comfortable to mentor trainee support staff and pharmacists.
- Salary IRO £43,000 - £54,000 p/a OR pro-rata
- 6 weeks holiday + BH (FTE OR pro-rata)
- MUST be a qualified Pharmacist in the UK with GPhC registration
- MUST hold their Independent Prescriber qualification
- MUST have Primary Care experience
