Lead Practitioner (Drug & Alcohol Services)

Pay Rate: 15 per hour
Hours: Monday - Friday, 9:00 AM - 5:00 PM
We are looking for a dedicated and experienced Lead Practitioner to join our team in providing high-quality support to individuals within our drug and alcohol services. This is an exciting opportunity to work with a diverse case load of 20-35 individuals, providing vital support through outreach work, 1:1 case management, and group sessions.
Key Responsibilities:
- Manage a caseload of 20-35 individuals, providing comprehensive support through outreach, one-to-one sessions, and group work.
- Conduct care plan reviews and risk assessments to ensure the safety and well-being of clients.
- Develop tailored support plans to meet the individual needs of clients and help them achieve their personal goals.
- Provide crisis intervention and safeguarding support as required.
- Work closely with other professionals and agencies to ensure holistic and person-centred care.
- Maintain accurate and up-to-date case notes and documentation in line with service requirements.
Key Requirements:
- Experience working within drug and alcohol services or a similar care environment.
- Strong case load management skills and experience in delivering care plans and risk assessments.
- Ability to engage and build positive, trusting relationships with individuals who may face complex challenges.
- Knowledge of safeguarding procedures and how to handle sensitive situations with professionalism and compassion.
- Excellent communication skills, both written and verbal.
- Ability to work independently and as part of a multidisciplinary team.
