Job Title: Rapid Assessment and Resettlement Practitioner
Location: Enfield
Hours Per Week: 36 hours
Rate of Pay: 20-21 p/h (Umbrella)
Enfield are currently searching for an experienced Rapid Assessment and Resettlement Practioner to join their Out of Hours team.
Main Duties:
- Conduct comprehensive assessments and develop Resettlement Plans and Personal Housing Plans for individuals who have experienced street homelessness.
- Collaborate with Specialist and Housing Coordinators to ensure effective implementation and delivery of housing solutions and support services.
- Maintain accurate and up-to-date service user documentation and enter relevant data into IT systems.
- Provide support and practical assistance to service users, working as part of a team to meet their needs and achieve positive outcomes
The Candidate Will Have:
- Experience working with people who have experienced homelessness, with a focus on resettlement planning and housing support.
- Knowledge of relevant housing and homelessness legislation, policies, and procedures.
- Ability to work flexibly, including shift work (nights and weekends), and collaborate within a multi-disciplinary team to meet service goals.
