Hatched Recruitment is working closely with the local authorities and we are looking for a Care Co-ordinator in the Enfield area.

We are looking for individuals who are passionate about working with people who have learning disabilities and their families. If you are committed to supporting people with learning disabilities to live autonomous lives and achieve positive wellbeing, then we want to hear from you.

This is an exciting opportunity to be based within a local authority integrated health and social care service. The service comprises of many disciplines including Social Work, Community Nurses, Psychologists, Occupational Therapists, Speech and Language Therapists, Art Therapists, Physiotherapists and Psychiatrists. The Care Coordinator post will sit within the social work team.

The role entails face to face work with service users, their families, and external partners. The purpose of the Care Coordinator role is to identify whether support packages continue to meet the needs of service users and their families. The role will require the successful candidates to make recommendations regarding support packages and arrange and amend support to ensure needs are being met in a person centred and strength-based way.

You will need excellent communication skills, both verbal and written. The Care Coordinator will be required to attend multi- disciplinary meetings and present cases, communicate effectively with service user's and their families as well as a range of internal and external colleagues and partners. In addition, the Care Coordinator will be required to write accurate, comprehensive support plans and assessments.
