Responsibilities include:
- Conduct detailed assessments to identify clients' mental health needs.
- Perform mental capacity assessments in line with the Mental Capacity Act 2005, and submit applications for Deprivation of Liberty Safeguards (DoLS) when appropriate, ensuring legal compliance and safeguarding individuals' rights.
- Provide immediate support and intervention in emergency and crisis situations.
- Collaborate with a multidisciplinary team, including psychiatrists, psychologists, nurses, and other healthcare professionals, to deliver coordinated, holistic care.
- Maintain accurate, clear, and up-to-date records of assessments, interventions, and care plans, ensuring all documentation adheres to council policies and legal requirements.
Requirements:
- Relevant social work qualification
- 2 experience in adult social work, including assessment and care planning.
