About the Role

We are seeking an experienced and dynamic Band 7 Physiotherapist to join the Home First Service. This role involves providing exceptional patient-centred care within a collaborative and supportive team environment.

As a key member of the Home First Service, you will:

  • Attend ward/board rounds and liaise effectively with acute and community services.
  • Perform follow-up Discharge to Assess (D2A) visits in the community to ensure seamless patient transitions.
  • Deliver comprehensive physiotherapy assessments and interventions across community and inpatient settings.
  • Contribute to the discharge planning process, ensuring safe and timely transitions of care.
Essential Requirements
  • HCPC Registration as a Physiotherapist.
  • Car Driver Essential: The ability to drive is mandatory due to the nature of the role.
  • Proven experience in community, inpatient, and D2A settings.
  • Strong communication and interpersonal skills, with the ability to build relationships across multidisciplinary teams.
  • Organised and adaptable, with the ability to manage a varied caseload in a fast-paced environment.
Working Hours
  • 37.5 hours per week over 4 days, initially Monday to Friday.
  • Potential for weekend working as the service evolves
