An exciting opportunity has arisen for a qualified and experienced Band 6 Physiotherapist to join a community-based team in Exeter, working within the Rehab or Urgent Care Team. This is a full-time position, but part-time applicants will also be considered. The role offers an excellent hourly pay rate of £25 - £26 per hour, with the opportunity to make a meaningful impact within the community.

Key Responsibilities:

  • Urgent Care Physiotherapy: In this role, the Physiotherapist will work closely with individuals requiring urgent physiotherapy interventions due to acute injuries, post-surgery recovery, or sudden exacerbations of existing conditions. The role includes assessing and treating patients with complex needs in their homes or other community settings, with a focus on rapid recovery, functional rehabilitation, and preventing unnecessary hospital admissions.
  • Rehabilitation Physiotherapy: Working within the Rehab Team, the Physiotherapist will support patients recovering from illness, injury, or surgery, helping them regain their independence and improve mobility. This will include providing rehabilitation following strokes, fractures, musculoskeletal injuries, and neurological conditions. The role focuses on long-term recovery and improving quality of life for individuals who require ongoing support.
  • Provide high-quality physiotherapy assessments and treatments for patients within the community.
  • Work as part of the Rehab or Urgent Care Team, ensuring seamless care for individuals with a range of rehabilitation or urgent care needs.
  • Develop and implement tailored rehabilitation programs, addressing both acute and chronic conditions.
  • Deliver evidence-based physiotherapy interventions aimed at improving patients' mobility, function, and quality of life.
  • Work collaboratively with multidisciplinary teams, including occupational therapists, social workers, and nurses, to ensure comprehensive patient care.
  • Manage a varied caseload of patients with complex needs, ensuring effective time management and prioritisation of patient care.
  • Provide support, education, and advice to patients and their families to enhance recovery, prevent hospital readmissions, and ensure the safe management of care at home.
  • Monitor and evaluate patient progress, adapting treatment plans accordingly to ensure continued improvements in functional outcomes.

Requirements:

  • HCPC registered Physiotherapist.
  • Experience working in a community-based setting, ideally within rehab or urgent care teams.
  • Expertise in assessing and managing a range of conditions, including neurological, musculoskeletal, and respiratory conditions, in the home or community environment.
  • Ability to work autonomously while managing a diverse caseload of complex patients.
  • Strong communication skills, with the ability to collaborate effectively with a multidisciplinary team.
  • A compassionate, patient-centred approach to care delivery, with a focus on promoting independence and quality of life for patients.
  • Ability to manage and adapt treatment plans for patients with varying needs, ensuring that interventions are evidence-based and appropriate for their condition.
