We’re searching for an experienced, compassionate, and proactive Senior Nurse specializing in Primary Care and Substance Misuse Services (SMS). Could this be your next career move?

Position: Band 6 Primary Care and SMS Senior Nurse
Location: Exeter
Duration: 3 months 
Schedule: Monday - Sunday (07:30pm-21:00pm)
Start Date: ASAP
Pay Rate: £27-£30 p/h

A Typical Day in This Role

As a Band 6 Primary Care and SMS Senior Nurse, you’ll play a vital role in supporting individuals with complex needs, including substance misuse and associated health challenges. Your responsibilities will include:

  • Conducting holistic assessments to understand patients' physical, mental, and social health needs.
  • Developing and implementing care plans tailored to address addiction management, harm reduction, and physical health issues.
  • Providing evidence-based interventions, including medication management and motivational interviewing.
  • Collaborating with multidisciplinary teams, community organizations, and external agencies to ensure cohesive care and support.
  • Acting as a key liaison for patients transitioning between services, ensuring smooth handovers and continued support.
  • Delivering education and advice on harm reduction, health promotion, and relapse prevention strategies.
  • Monitoring and responding to safeguarding issues with urgency and care.
  • Maintaining accurate, timely documentation in line with professional and organizational standards.
What We’re Looking For
  • Qualifications: Current NMC registration as a Registered Nurse (Adult or Mental Health).
  • Experience: Substantial experience in primary care, substance misuse, or addiction services.
  • Skills: Strong communication, advocacy, and leadership capabilities.
  • Approach: A compassionate, patient-centered approach to care, coupled with the ability to work collaboratively in a fast-paced environment.
Working Hours and Duration

This role offers flexible shifts across Monday to Sunday, providing adaptability to fit your schedule. The initial contract will be for 3 to 6 months, with potential for extension based on performance and service needs.
