Job Overview

We are seeking an experienced Band 6 Physiotherapist to join the Intermediate Care Team in Newton Abbot. The role involves delivering rehabilitation services aimed at promoting independence and preventing hospital admissions. This is an excellent opportunity to work collaboratively with a multidisciplinary team in a dynamic and supportive environment.

Key Responsibilities
  • Conducting comprehensive assessments of patients referred to the Intermediate Care Team.
  • Developing and implementing individualized rehabilitation programs to achieve optimal patient outcomes.
  • Utilizing evidence-based techniques to support recovery, improve mobility, and enhance quality of life.
  • Liaising with multidisciplinary teams, including occupational therapists, nurses, and social workers.
  • Providing guidance and education to patients and their families on treatment plans and preventative care.
  • Participating in regular team meetings and contributing to service development initiatives.
Key Requirements
  • Qualifications: Degree in Physiotherapy and HCPC registration.
  • Experience: A minimum of 2 years post-qualification experience, with a focus on rehabilitation, community care, or intermediate care.
  • Skills: Excellent communication, clinical reasoning, and patient management skills.
  • A valid UK driving license and access to a vehicle for community visits.
