What You’ll Be Doing:Join our team as a Band 6 Community Physiotherapist and make a real difference in the lives of patients in the local community. In this role, the successful candidate will:

  • perform holistic and person-centred assessments in patients’ homes or community settings
  • create and implement tailored rehabilitation and treatment plans
  • promote independence, mobility, and safety in the home environment
  • work closely with multidisciplinary teams including OTs, nurses, and social care professionals
  • support hospital discharge and admission avoidance through timely intervention
  • maintain accurate, up-to-date patient documentation

Requirements & Qualifications:To thrive in this role, the ideal candidate will have:

  • current and valid HCPC registration
  • experience in community physiotherapy or working with elderly and frail populations
  • excellent clinical reasoning and problem-solving skills
  • the ability to work independently in the community (a car driver is typically essential)
  • a commitment to providing compassionate, high-quality care

Working Hours & Contract Duration:This flexible position is primarily Monday to Friday, full-time, but part-time applicants are welcome. The role is expected to begin in April or May with an ongoing contract and potential for extension depending on service needs.
