Job Overview:We are seeking a skilled Band 6 Occupational Therapist to join Torbay and South Devon NHS Foundation Trust. This role involves working within the community to provide high-quality Occupational Therapy services to patients with diverse health conditions. The successful candidate will assess patients, develop tailored rehabilitation plans, and support individuals in regaining independence within their homes and communities.
Responsibilities:- Conduct comprehensive Occupational Therapy assessments and create personalised treatment plans.
- Provide rehabilitation, advice, and equipment to enhance patients' daily living and mobility.
- Work collaboratively with multidisciplinary teams, including physiotherapists, nurses, and social care professionals.
- Support hospital discharge planning by facilitating safe transitions to community settings.
- Educate patients and caregivers on strategies to improve independence and quality of life.
- Manage a caseload efficiently, prioritising patient needs and adapting care plans accordingly.
- HCPC-registered Occupational Therapist with relevant post-registration experience.
- Minimum of six months NHS experience in a community or hospital setting.
- Strong clinical reasoning and problem-solving skills.
- Ability to work independently and as part of a multidisciplinary team.
- Experience in discharge planning and working with complex care needs is desirable.
