Collaborate with CAMHS, children’s services, and adult mental health teams to create and implement smooth transition plans.
Advocate for young people’s needs by developing personalised care plans to promote their independence and well-being.
Engage with families and carers to support their involvement in the transition process.
Conduct comprehensive assessments to identify support needs and help develop community engagement and independent living skills.
Work within Care Act principles to ensure young people can gain employment and live independently.
Manage a varied caseload, maintaining up-to-date records of interventions and outcomes.
