We are looking to recruit a Qualified Social Worker within a Permanency & Transitions Team within the Social Care industry in Devon County Council. The successful candidate will be working in a well-structured, organised team who are growing their social worker staffing levels

Main duties include:

  • Befriending, advising, assisting and supporting young people leaving the care of the local authority and moving onto independence as per the Children Leaving Care Act 2000
  • Reviewing or supporting the review of the pathway plan, and when necessary amending previously agreed recommendations set out in the young person's Pathway Plan
  • Working directly with care leavers, aged 16 plus, to facilitate their transition to adulthood and independence from local authority care, ensuring the young people's objectives that are set out in their Pathway Plan are met
  • Developing close and effective working relationships with key statutory and voluntary agencies and partners to facilitate a planned and coordinated multi-agency response to the identified needs of care leavers
  • Preparing and maintaining appropriate written records
  • Taking part in a duty system
  • Developing and sustaining good working relationships with young people

To be successful:

For this role you will need to be a Qualified Social Worker with a SWE Registration and educated to Degree Level. Candidates will need to have an excellent knowledge of therapeutic approaches, relevant legislation, national policy and initiatives and experience working with Children and Families. You will also need to have the Eligibility to Work in the UK with previous experience within Adult Social Care Team.
