About the Role
We are looking for a Referral and Assessment Manager to oversee referrals and assessments for individuals with Learning Disabilities (LD) and Autism. You will manage the smooth transition of service users into their new homes and support their movement across services.

Key Responsibilities

  • Manage and assess incoming referrals to ensure the best fit for our services.
  • Work closely with families, authorities, and healthcare professionals.
  • Maintain accurate records of referrals and placements.
  • Ensure compliance with all regulatory requirements.
  • Build strong relationships with commissioners and other key stakeholders.

Skills & Experience

  • Experience in referrals, assessments, and placements within LD and Autism services.
