Our client is on the lookout for a passionate and proactive Care Coordinator for this Domiciliary Home Care Branch ready to lead from the front, in delivering exceptional, person-centred care.

Role Overview

As Care Coordinator - you will be supporting carers, coordinating care and working hand-in-hand with the Branch Manager and integrated care services, to ensure smooth, compassionate service delivery. Primarily community-based, the position will include office-based work as the branch expands.

Key Responsibilities

  • Deliver hands-on care in the community
  • Create & implement individualised care plans
  • Organising your shifts, administering and recording medication
  • Liaising with health professionals and maintaining residents' computerised care plans
  • Assisting with induction, training and supervision of support staff

Requirements

  • NVQ3 Health & Social Care or willing to work towards
  • Experience of supervising a care team
  • Empathy, organisation & leadership skills
