We have partnered with one of the leading GP practices in Hampshire which has a list size of approx. 9000 patients. They have a clinical diverse clinical team who are dedicated to providing the best care for their patients

Position Details:

  • Job Title: Practice Nurse.
  • Job Type: Full time or would consider part-time.
  • Location: Hampshire (near Farnborough)
  • Salary: £38,000 - £46,000 per annum with an extensive benefits package including personal development opportunities (CPD leave entitlement) and much more.

Main Duties:

  • Identify, and manage as appropriate, treatment plans for patients both at risk of developing a long-term condition and diagnosed chronic disease.
  • Support the Infection Prevention Control (IPC) lead with the implementation of audits, training, support and evidence of compliance to the practice manager.
  • Develop, implement and embed an effective care management programme for the frail and housebound patients registered at the practice.
  • Develop, implement and embed health promotion and wellbeing programmes.
  • Implement vaccination programmes for adults and children.

About You:

  • Registered Nurse with experience within a GP setting.
  • Experience in chronic disease management inc. diabetes, hypertension, and respiratory conditions.
  • Imms/Vacs trained, inc childhood imms.
  • Experience using EMIS is desirable but not essential as full training provided.
